I cannot express my gratitude enough for the exceptional care that Unique Hospice provided to my loved one during their final days. The team of nurses and staff were all incredibly kind, compassionate, and dedicated to ensuring that my loved one was comfortable and well-cared for. They were always available to answer any questions or concerns that we had, and provided us with the support and guidance that we needed during such a difficult time.

Unique Hospice is truly special. They go above and beyond the Medicare requirements, and work with each patient's particular needs and comfort in their last days. They are extremely responsive, and are always willing to support. I run a RCFE and they never let me down when I need them. They understand our limitations and support us in providing the BEST and necessary care for our residents.

I'm sure in the field you are in, you frequently receive letters thanking you for the services you provide during the terrible times that hospice services usually entail. We are coming up on one year since my grandmother passed, and while the grief still lingers, what does also is the gratitude that we had a hospice service such as yours to help us through it.

The office staff was always there with all my questions and concerns I had regarding my mom. They were so patient and kind every time I called and it never made me feel like a bother. They always had the answer or would get back to me if research was required.
I would like to thank the nurses for the visits and the weekly reports on her condition in a professional, yet cheerful manner. Recently a nurse notified me of a "change in condition" in my mom's health. During a visit I noticed she appeared to be struggling and in pain. Under hospice care, my mom was given pain medication immediately which helped her considerably.
I am so grateful for the services you provided, the positive interactions we had, and all the people behind the scenes. She would not have left this earth as gently as she did without Unique Hospice.

Unique Hospice took care of my grandma, providing the best comfort for her before her last days with us. Our nurse, Cassandra, was very knowledgeable, skilled and professional in what she does and I can tell how passionate she is in providing the best care to my grandma. She made the entire family confident that my grandma is receiving the proper care when she only had a few days left of life expectancy left. She also calmed us down and told us that there are no "what if's" to change anything besides having to accept it and have my grandma die peacefully. It is heartbreaking to lose someone who served as your role model, but seeing them suffer is even more painful. Nevertheless, I was relieved that she passed away gracefully at home with her family surrounding her after she received care from Unique Hospice.

Hospice and Palliative care is very touchy. Having a loved one in such a vulnerable place is very hard. They do an amazing job taking care of their patients and putting the family at ease, as much as such a thing is possible. Exceptional, compassionate, and attentive - Unique Hospice provided my dad with personalized care during his final days. The team was supportive, knowledgeable, and approachable throughout the entire process. I highly recommend them for anyone seeking exceptional hospice care.

This agency was an absolute blessed foundation that supported my family and I thru our most difficult and vulnerable times, not once but three times for that matter; with my father twice (Palliative and Hospice) and my grandfather (Hospice). As a healthcare provider, it was even more so to have to met/exceed my clinical expectations. The entire staff was on point, and professional with their genuine caring hearts. My family and I would not of been able to deal with the sensitive health circumstances and family affairs without Unique Hospice's support. Unique Hospice will forever be our extended family. Unique Hospice is an organization staffed with competent, knowledgeable, and caring individuals. Their 24 hour phone line is truly a wonderful tool. The 24 hour staff answer questions with patience, competency, and concern, regardless of the time of day or night! They return calls promptly and treat every situation with a calm and quiet demeanor that encourages confidence and assurance for the patient's family. Our case nurse Johanna was truly a gift to the patient and the family!

i was searching for a hospice for my 92 years old mom who has severe dementia. i spent a lot of time reading about different companies who provide service in the san gabriel valley area. after picking 3, i started the interviewing process. i was so impressed with vanessa on the phone, who tirelessly answered all my questions, and set up the NP visit at my home promptly. my husband and i were even more impressed with the professionalism and warmth NP adi who did a quick, but thorough check up on my mother, i decided to sign with them immediately. the following day, the visiting RN geo performed the routine check up; he was a kind, gentle young man who made my mom so comfortable, she had no problem cooperating! it has been 9 days since we signed up with unique hospice service. we also found and moved mom into an assistant living home with unique's help. i was in constant communications with them every day, including weekends. everyone was always professional, pleasant and a problem solver. i am so pleased with the nurse, geo, i request that he would be assigned to take care of mom on the 3 weekly visits at the assistant living! unique hospice and all the people working there were real angles to me and my family during this most difficult time dealing with the last days of mom. they made me feel at peace, because i know mom is in great care! thank you!

I don't have enough words to say about the care my grandmother received from Unique hospice. I'm someone that has a question for everything and with each question I was greeted with love any care. My grandmother was under their hospice care for 5 months and I'm forever grateful for each of the staff that worked with my grandmother. Jeni gave the best bed baths to my grandmother. She was so patient and caring not to mention the sweetest lady. Stacie and Cassandra, the two Lvn's that worked with my grandmother revived so many calls from me. I never once felt like a bother. They both were always so loving and willing to go the extra mile to make sure my grandmother was taken care of. During my grandmothers last breaths Stacie even stayed by my side so I wouldn't be alone. Adi, the NP was already willing to take my questions. I often would want to process the choices I was making with her. She always took her time with me to make sure I was making the most informed choice I could. And Aberham, the owner of the company, checked in with me often to make sure we had everything we needed. The last days before my grandmother passed I didn't want to leave her side. I was brought a recliner to sleep in right next to her. I'm forever grateful for each of the staff at Unique. All of these people become family in a time I felt so alone. So grateful for each of them!
